Think of it as the foundation of your entire application. Without this form, you cannot schedule your interview at the U.S. Embassy. It is the legal proof that you have been vetted and are ready to train in the USA.
You no longer need to wait for international mail! The U.S. government now allows sponsors to email this form digitally. You simply print, sign, and go.

Your DS-2019 contains four critical pieces of information.
Verify these immediately upon receipt.
Located in the top right corner. This is your unique ID in the U.S. system. It always starts with N (e.g., N0012345678).
Box 3 shows your exact Start and End dates. You cannot work outside of these specific dates—no exceptions.
This lists the U.S. Dept. of State sponsor (our partner) vetting you. This is legally distinct from your Host Company.
Ensure this matches your application (e.g., “Intern” vs “Trainee”). This defines your maximum stay duration.
Treat this form like your passport. Keep it safe, keep it flat, and never lose it! You will need to present the original (or signed print-out) at the airport border control.
Don’t panic. Contact your sponsor immediately to get it corrected before your embassy appointment.
You cannot simply “download” this form. It is a controlled legal document that must be issued by a designated sponsor.

The sponsor vets you, approves the application, and emails you the digital DS-2019.
You use the unique SEVIS ID on your form to book your visa appointment.
Mistakes on your DS-2019 can cause visa delays or denials. Review these three details immediately.
Does your name match your passport exactly? Check every letter.
Warning: Even one wrong letter can cause a denial.Do the Start Date and End Date match your official internship offer letter?
Are you listed correctly as an “Intern” or “Trainee”?
(Interns are students/recent grads; Trainees are experienced pros).

The Form DS-2019, also known as the Certificate of Eligibility for Exchange Visitor (J-1) Status, is the official document that allows you to apply for a J-1 Visa at a U.S. Embassy or Consulate. This form is issued only by organizations authorized as sponsors by the U.S. Department of State. Sponsor organisations review applications, verify eligibility, and then produce the DS-2019 form for selected participants. With this form, you can apply for a J-1 visa at your local U.S. embassy or consulate. Approval is determined by consular officers who analyze your application, documentation, and overall qualifications.
The DS-2019 (Certificate of Eligibility) is the primary document used to track J-1 exchange visitors. It serves three critical purposes:
It is the single most important document in your application. The U.S. Embassy cannot process your visa without the unique SEVIS ID number located in the top right corner of this form. It effectively tells the government, “This person has been vetted, has insurance, and has a valid training plan.”
You cannot download a blank DS-2019 from the internet. It must be issued to you by a U.S. Department of State-designated sponsor. At Bridge Aspire, we guide you through the placement process and connect you with trusted sponsors who will issue this form once your application is approved.
The process is straightforward with Bridge Aspire:
We help you secure a placement with a U.S. Host Company.
We assist you in submitting your application to a designated sponsor.
Once the sponsor approves your training plan (DS-7002) and qualifications, they will issue and email you the DS-2019.
You do not fill out this form. Your designated visa sponsor fills out all the data based on your application. Your only job is to review it for accuracy, print it, and sign your name at the bottom in blue or black ink.
There is no public “version” to find. The form is generated dynamically by the U.S. government’s SEVIS system specifically for you. Your sponsor is the only entity that can generate this document for you.
Think of SEVIS (Student and Exchange Visitor Information System) as the live government database, and your DS-2019 as the “receipt” from that database. Your DS-2019 contains your unique SEVIS ID (starts with N00…), which links your physical paperwork to your digital government record.
Yes. Your form is valid only for the dates listed in Box 3 (Form Covers Period).
Start Date: You cannot start working before this date.
End Date: You must stop working on this date.
Grace Period: You have 30 days after your end date to travel within the U.S., but you cannot work during this time.
Yes, but with conditions. If you leave the U.S. during your internship and plan to return, your DS-2019 must have a valid Travel Signature (Validation) from your sponsor in the bottom left corner. This signature confirms you are still in good standing with your program.
If you spot a typo (like a misspelled name or wrong birthdate), do not write on the form yourself. Contact your sponsor (or your Bridge Aspire advisor) immediately. The sponsor will update the information in SEVIS form and issue you a corrected DS-2019. You must have the corrected form before your visa interview.
The most common issues we see are:
Name Mismatch: The name on the form doesn’t match the passport exactly (e.g., missing a middle name).
Wrong Category: Being listed as an “Intern” when you are actually a “Trainee” (or vice versa).
Forgeting to Sign: You must sign the bottom of the form before your embassy interview.
You will need the DS-2019, DS-7002, and I-901 Receipt for your embassy interview.
